Seniors at Risk in Hamilton
A collaboration between Good Shepherd, Alzheimer Society and St. Matthew’s House.
Facilitating timely interventions and support for seniors impacted by dementia, abuse, mental and physical health challenges, precarious housing, food insecurity, social isolation and more.
A new collaborative approach to care
Our Services
Case Management and Advocacy

- Timely case coordination and intervention
- Connecting and collaborating with community agencies
- Advocacy and education especially those experiencing abuse
- Assist with housing & benefits applications. Income tax filing etc
Dementia and Caregiver Support

- Support to persons living with dementia and their caregivers, so they can live safely in their community
- Individually tailored care plans based on our collaboration with caregivers and patients
Homelessness Prevention and Intervention

- Provide assistance and support that address housing and hoarding issues eviction prevention
- Legal support pertaining to landlord tenant board issues
- Connecting vulnerable seniors to emergency food and prepared meals

In consultation with Ontario Health West, the Alzheimer Society, Good Shepherd and St. Matthew’s have implemented a new collaborative approach called Seniors at Risk in Hamilton (SaRiH).

Virtual Seniors Group
Aims to connect seniors in our community virtually once a month via Zoom. Many older adults in our communities are socially isolated and unable to access in-person program due to physical ,transportation difficulties and other barriers.

Volunteer Telephone Support
Welcome to our interactive programs for your mental, social and emotional well-being. These programs are delivered entirely over the telephone by peer volunteers and there is no cost for participants. This free service is available for older adults who are 55+.

Contact us or make a Referral
Referrals to SaRiH are managed through a central access point and reviewed by representatives from Good Shepherd, the Alzheimer Society, and St. Matthew’s House. Each referral will be assigned to the appropriate SaRiH partner.
